How Acupuncture Releases Tissue Adhesions: The Shear Force Mechanism침이 조직의 유착을 푸는 원리: 전단력(Shear Force) 기전

There are several mechanisms behind how acupuncture may work, and we've already covered one of them, mechanotransduction and immune signaling, in a previous post. (How Acupuncture Works: The Science of Mechanotransduction and Immune Signaling) Today, let's talk about another one: how acupuncture may physically release adhesions between tissue layers, and why that matters for nerve conduction and fluid circulation.

침의 기전에는 여러 가지가 있는데요, 지난 글에서는 기계적 신호전달과 면역 반응에 대해 다뤘습니다. 오늘은 또 다른 기전, 침이 조직 간의 유착(adhesion)을 물리적으로 해리시키고, 이를 통해 신경전달과 체액순환을 원활하게 하는 과정, 에 대해 이야기해보겠습니다

There's a reasonable body of evidence suggesting that releasing physical adhesions between tissue layers with acupuncture may produce effects similar to manual adhesion release techniques used in physical therapy and manual medicine, improving how freely tissue layers glide against each other, which in turn may support nerve conduction and fluid (blood and lymph) circulation.

조직 간의 유착을 해리하여 신경전달과 체액순환을 원활하게 하는 효과를, 침으로도 비슷하게 낼 수 있다는 나름의 근거가 있습니다.

The Mechanism: Friction, Shear Tension, and Tissue Restructuring- how acupuncture releases adhesion

기전: 마찰력, 전단 장력, 그리고 조직의 물리적 재구성

When a practitioner moves the needle up and down inside the tissue, a common manipulation technique in acupuncture, friction occurs at the point where the needle meets the soft tissue. This friction generates what's called shear tension: a sideways mechanical force that physically changes the structure of the surrounding soft tissue. In practical terms, this is a mechanical un-sticking of tissue layers that have become adhered, or "stuck," to one another over time.

침을 시술자가 조직 안에서 위아래로 움직이는 동작(침구 치료에서 흔히 쓰이는 수기법)을 할 때, 침과 연부조직이 만나는 지점에서 마찰이 발생합니다. 이 마찰이 '전단 장력'이라는 것을 만들어내는데, 쉽게 말해 옆 방향으로 작용하는 물리적인 힘이 주변 연부조직의 구조를 실제로 변화시키는 것입니다. 쉽게 설명하면, 시간이 지나며 서로 달라붙어 있던 조직층이 물리적으로 떨어지며 풀리는 과정입니다. how acupuncture releases adhesions

This builds directly on Dr. Helene Langevin's foundational research. In a follow-up to her 2001 mechanotransduction paper, Langevin and Yandow published "Relationship of Acupuncture Points and Meridians to Connective Tissue Planes" (The Anatomical Record, 2002), mapping acupuncture points against cadaver dissections and finding a strong correlation between traditional point/meridian locations and the body's connective tissue planes and fascial septa. In that same body of work, the sensation of de qi (득기), the characteristic ache or "grasp" sensation patients feel when a needle is manipulated correctly, was described as the physical result of collagen fibers winding around the needle.

Why Friction Matters, and Why More Isn't Always Better

마찰력이 중요한 이유, 그리고 무조건 많다고 좋은 것은 아닌 이유

Because generating friction is central to this effect, two factors increase it: a thicker needle creates more surface contact, and repeating the up-and-down movement more times increases the mechanical effect. Both of these increase how much tissue restructuring can occur.

마찰력을 만들어내는 것이 이 효과의 핵심이기 때문에, 두 가지 요소가 이를 좌우합니다: 침이 굵을수록 접촉면이 넓어져 유리하고, 침을 위아래로 움직이는 횟수가 많을수록 물리적 효과가 커집니다. 두 요소 모두 조직 재구성 효과를 높입니다.

But this comes with an important caveat. If the procedure causes significant pain, soft tissue (skin, muscle, and surrounding structures) tends to guard and tense up in response, which can work against the intended effect rather than support it. In other words, pushing harder on friction without regard for the patient's pain response can backfire.

하지만 여기에는 중요한 전제가 있습니다. 시술 통증이 심하면 오히려 피부, 근육 등 연부조직이 긴장하며 방어적으로 수축하게 되고, 이는 의도한 효과와 반대로 작용할 수 있습니다. 즉, 환자의 통증 반응을 고려하지 않고 마찰력만 높이려 하면 오히려 역효과가 날 수 있다는 뜻입니다.

This is why technique and clinical judgment matter as much as the underlying mechanism. One approach we favor is using several very thin needles, inserted in a way that minimizes insertion pain, to achieve a comparable therapeutic effect without triggering the tissue-guarding response that comes with a more forceful, painful technique.

그래서 기전만큼이나 시술자의 기법과 임상적 판단이 중요합니다. 저희가 선호하는 접근 중 하나는, 여러 개의 아주 가는 침을 삽입 시 통증을 최소화하는 방식으로 사용하여, 통증을 유발하는 강한 자극 없이도 비슷한 치료 효과를 내는 것입니다.

How We Apply This in Practice

At Lena Kim Acupuncture, when we're specifically targeting adhesions, for example, in post-injury scar tissue, restricted fascia after surgery, or areas of chronic muscular tension where tissue layers have stopped gliding freely, we select needle gauge and manipulation intensity based on what the tissue and patient can tolerate, always weighing therapeutic friction against the risk of triggering guarding. This is also part of why we combine multiple thinner needles rather than relying on aggressive single-needle manipulation alone.

Patient Case Study

환자 사례

A woman in her early 50s came to us with a frozen shoulder (adhesive capsulitis) that had persisted for several months, with significant restriction in overhead and rotational movement. Rather than using an aggressive single-point manipulation approach, we used multiple thin needles around the shoulder capsule and along the affected fascial planes, with a gentle up-and-down needle movement adjusted to her pain tolerance. Over six weekly sessions, her range of motion in external rotation improved measurably, and she reported being able to reach overhead shelves without the sharp catching sensation she'd had at the start of treatment.

50대 초반 여성 환자분이 몇 달째 지속된 오십견(유착성 관절낭염)으로 내원하셨습니다. 팔을 위로 들거나 회전하는 동작에 상당한 제한이 있었습니다. 강한 단일 자극 방식 대신, 어깨 관절낭 주변과 관련 근막 부위에 여러 개의 가는 침을 사용하고, 환자분의 통증 내성에 맞춰 침을 부드럽게 위아래로 움직이는 방식을 적용했습니다.6주간의 주간 치료 후, 외회전 가동범위가 측정 가능할 정도로 개선되었고, 치료 초반에 있었던 날카로운 걸리는 느낌 없이 위쪽 선반에 손이 닿을 수 있게 되었다고 보고하셨습니다.

Safety and What to Expect

안전성 및 치료 시 기대할 수 있는 것

Because this mechanism relies on friction and manipulation, sensation during treatment can range from a mild ache to a more noticeable pulling or heavy sensation, this is generally a normal part of the process rather than a sign of injury. That said, if a technique causes sharp or significant pain, we adjust immediately, since (as discussed above) pain-driven tissue guarding can undermine the intended effect. As with any adhesion-focused work, results tend to build gradually over a series of sessions rather than resolving in a single visit.

Lena's Note

Something I think about often in practice: more force isn't always more effective. This mechanism gives me a concrete way to explain why. Friction and shear tension do the work, but pain-driven guarding can cancel it out. That tells me the real skill isn't picking one "best" technique, it's accurately reading each patient's tissue and pain tolerance in the moment, and adjusting needle choice and manipulation accordingly. The mechanism is the same for everyone; the right approach for a given patient almost never is.

임상에서 자주 생각하는 것 중 하나는, 힘이 세다고 항상 더 효과적인 것은 아니라는 점입니다. 이 기전은 그 이유를 구체적으로 설명해줍니다. 마찰력과 전단 장력이 실제로 작용하지만, 통증으로 인한 조직 긴장이 그 효과를 상쇄할 수 있습니다. 그래서 저는 정해진 하나의 "정답 기법"을 고르는 것보다, 그 순간 환자분의 조직 상태와 통증 내성을 정확히 파악하고 그에 맞춰 침의 선택과 수기 강도를 조정하는 것이 진짜 실력이라고 생각합니다. 기전 자체는 누구에게나 동일하지만, 그 환자에게 맞는 방법은 거의 매번 다릅니다.
